If you have previously ordered from Canadian Pharmacy Choice- you must fax or mail your orders to us using this form to download and fill out manually in doc , PDF or HTML , or fill out the form below- and print out the next page with all the answers generated and fax or mail that the form in.

We will commence shipment to your door or PO Box once we receive your payment.
